The Discharge Nurse is accountable for the delivery of safe patient care utilizing the nursing process. The Discharge Nurse assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient needs. The Discharge Nurse develops a thorough, safe plan of care that encompasses all aspects of the patient's care needs at the time of discharge from the hospital. The Discharge Nurse contributes to the support of patients through interdisciplinary teamwork. The Discharge Nurse is accountable to assure the care provided is consistent with applicable standards, regulations, statutes and state, federal law. The Discharge Nurse serves as a resource for other staff and clinicians as required.
