About The Position

The Hospice of Baton Rouge is seeking a skilled, dedicated, and detail-oriented Admissions Coordinator to join our team. This role will be responsible for managing all clerical aspects of a patient's admission process, from initial intake to final admission, ensuring a smooth and timely experience for patients.

Requirements

  • High School Diploma or GED required; advanced education preferred.
  • Prior experience in a health care environment, home care, or hospice preferred.
  • Prior management experience preferred.
  • Exhibit a competent working knowledge in the following computer software programs: Microsoft Word and other basic Window applications.
  • Exhibit excellent interpersonal, organizational, and verbal communication skills.
  • Meet or exceed the listed qualifications in the LA Minimum Standards for Hospice.
  • Must be physically able to fulfill all duties of the position unassisted.
  • Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
  • Installation of company communication application on cellular device.

Responsibilities

  • Maintain professionalism in all interactions with staff, patients, and families.
  • Answer and triage all intake calls. Document information regarding referral and communicate effectively to the intake team. Communicate all updates back to the referral source timely and accurately.
  • Responsible for completing the morning census email and updating the census boards
  • Review and edit the Admission Department reports timely.
  • Responsible for tracking referrals not yet admitted.
  • Assist the intake team with scanning completed checklists and referrals.
  • Provides backup coverage in intake as needed.
  • Oversees all administrative elements of the admissions process, working in coordination with the nursing and accounting departments.
  • Monitor all referrals that have not been admitted and participate in timely follow-up actions as needed.
  • Coordinate with social services/nursing/marketing to schedule information/admission visit
