About The Position

Ebenezer is looking for an Admissions Coordinator II to join our corporate admissions team at the Ebenezer Corporate Office in Edina, MN! This position will develop relationships with referral sources and work with internal teams to promote and maintain census. Assist Social Service Department in meeting the needs of the residents and families of the care center. This admissions schedule includes; On Call/Casual Day shifts available To maintain Casual status, employees must be willing to be available to work a minimum of three (3) shifts per month (two shifts must be weekend shifts) and one (1) Holiday shift per year. The weekend starts with the beginning of the night shift on Friday and ends with the beginning of the night shift on Sunday

Requirements

  • Associate of Arts Vocational/Technical Training or Associate Degree
  • One to three years of previous experience
  • Licensed Practical Nurse Licensed Practical Nurse (LPN) or Registered Nurse Registered Nurse (RN)

Nice To Haves

  • Bachelor of Science In Medical or Human Service Field
  • Three to five years of previous experience
  • 2 years experience in LTC
  • Computer Skills
  • Experience in Medical or Human Service Field

Responsibilities

  • Ability to process and complete admissions
  • Provides pre-admission counseling, information and tours
  • Clinically evaluates referrals to ensure all needs can be met from a medical perspective
  • Facilitates the intake process and secures primary care provider
  • Reviews EPIC for proper documentation and orders
  • Contacts pharmacy as needed to verify cost of meds
  • Determines need for equipment and supplies and coordinates with internal team
  • Facilitates PAS Screening and Level II as needed
  • Coordinates admission process with resident and resident representative, community providers and staff as appropriate
  • Sets up interpreter services as needed
  • Assures completion of admission paperwork
  • Informs residents and resident's representatives of financial responsibilities, payment options and agency contacts according to the state and federal regulations
  • Builds and maintains relationships with referral sources and assists with marketing and outreach activities as appropriate
  • Maintains internal and external waiting lists as appropriate
  • Verifies necessary payer source(s) as appropriate
  • Coordinates room change process as appropriate
