About The Position
Hospice Admission RN – $10,000 Sign-On Bonus Full-Time | Porter, LaPorte, Jasper, Lake, Starke, & Newton Counties Position Overview We are seeking a compassionate and skilled Hospice Admission Registered Nurse (RN) to join our growing team. In this role, you will complete admission assessments, educate patients and families, and help ensure a smooth transition into hospice services. This is a field-based position covering Porter, LaPorte, Jasper, Lake, Starke, and Newton Counties, offering autonomy, meaningful patient interactions, and the opportunity to make a profound impact.
Requirements
- Current RN license in the state of Indiana
- Minimum of one year nursing experience (hospice or home health preferred)
- Strong understanding of hospice philosophy and patient-centered care
- Valid driver’s license and reliable transportation
- Excellent communication and assessment skills
- Ability to work independently in a field-based environment
Responsibilities
- Conduct comprehensive hospice admission assessments
- Complete all admission documentation and consent forms
- Evaluate patient condition, safety, symptoms, and care needs
- Initiate the individualized Plan of Care
- Educate patients and families on hospice philosophy, disease progression, and symptom management
- Obtain necessary physician orders, medications, and DME
- Collaborate with interdisciplinary team members to ensure seamless care
