About The Position

Admission RN- ($10,000 Bonus) Valparaiso Description Hospice Admission RN – $10,000 Sign-On Bonus Full-Time | Porter, LaPorte, Jasper, Lake, Starke, & Newton Counties Position Overview We are seeking a compassionate and skilled Hospice Admission Registered Nurse (RN) to join our growing team. In this role, you will complete admission assessments, educate patients and families, and help ensure a smooth transition into hospice services. This is a field-based position covering Porter, LaPorte, Jasper, Lake, Starke, and Newton Counties , offering autonomy, meaningful patient interactions, and the opportunity to make a profound impact. Requirements

  • Current RN license in the state of Indiana
  • Minimum of one year nursing experience (hospice or home health preferred)
  • Strong understanding of hospice philosophy and patient-centered care
  • Valid driver’s license and reliable transportation
  • Excellent communication and assessment skills
  • Ability to work independently in a field-based environment

Responsibilities

  • Conduct comprehensive hospice admission assessments
  • Complete all admission documentation and consent forms
  • Evaluate patient condition, safety, symptoms, and care needs
  • Initiate the individualized Plan of Care
  • Educate patients and families on hospice philosophy, disease progression, and symptom management
  • Obtain necessary physician orders, medications, and DME
  • Collaborate with interdisciplinary team members to ensure seamless care
