The Admissions Team Manager is responsible for oversight of intake and referral processing services. This position provides oversight of team members’ performance, providing feedback, mentoring and education to contribute to staff’s development. The Manager will work in collaboration with the team to meet the needs and expectations of the medical community, patients and families while meeting the region’s key performance indicators. Quality Provision of Services: Owns admission availability resource calendar and ensures admission visit availability meets referral volume and conversion metrics. Enhances the integrity and quality of the team by providing them with a consistent role model, mentor, instructor and resource person. Assumes responsibility for own professional growth and development in order to maintain and improve competence. Evaluates own needs for support and uses identified system(s) to meet the need. Develops new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc. Participates in quality improvement activities, QAPI program and hospice sponsored in-service training. Monitors, analyzes, and collaborates with the Referral Center Leadership team to improve inquiry to start of care timeliness and effectiveness of scheduled appointment. Assists the Regional Directors with reporting and presentation of key metrics related to all aspects of admission outcomes. Provides clinical guidance, as needed, to the Admission Team RNs with complex cases and situations. Organization/Regulatory Compliance: Ensures compliance with all areas of federal, state, and local laws and regulations as well as CHAP accreditation standards, Conditions of Participation, and the organization’s standards of care, policies and procedures. Communicates with the appropriate physician(s) regarding the patient’s condition, incorporating hospice expertise. Implements physician’s orders by documenting and communicating to staff, patient/caregiver or facility. Utilizes appropriate resource services (ambulance, IV therapy, DME, inpatient facilities, and appropriate agencies) to implement orders. Performs the introductory (90 day) and annual performance review of assigned team members in accordance with the organization’s policy and timelines. Supports and assures that clinical documents meet the organization’s patient care and reimbursement regulatory standards. This is accomplished through use of tools such as: the Admissions Audit tool, IDT meetings and documentation. Assists in performing quality audits that assure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organization’s policies and procedures. People/Communications: Evaluates the effectiveness of daily schedules and assignments of the Admission Team RNs and professionals. Assists the Regional Director in assessing the need for new staff and to recruit, interview, and make hiring decisions, in collaboration with Human Resources. Orients new staff to the team after HR and clinical orientations. Coaches and counsels Admissions RNs as needed. Coordinates with the Clinical Education staff to assure that assigned staff attend and receive education programs. Creates, manages, and evaluates Admission Team clinical escalation protocols that ensure complex clinical decisions at the time of admission are escalated as needed. Identifies opportunities to improve delays in care, admission related internal communication, admission process, and make appropriate recommendations to Regional Director. Holds, at minimum, quarterly staffing meetings to review key performance indicators, quality scores and discuss organization and/or team updates. Financial Stewardship: Achieves expected productivity standard as defined by the organization and in alignment with best practices/the Capital Caring Health model. Assures accuracy and timeliness of clinical staff daily assignments and payroll entries.
