About The Position

Join our growing team as an admissions coordinator! Schedule Details: 1 full-time position (40 hours / week) Will be based at our Eau Claire – West location serving multiple Dove Healthcare locations Working as an admissions coordinator, you will be responsible for: Coordinating admissions with hospital discharge planners and other referral sources Assessing, coordinating and ensuring a smooth admission transition Confirms medical and financial information needed to ensure a smooth admission transition Develops and maintains excellent networking skills and relationship building with referral sources Ensuring patient continuity of care About Dove Healthcare Dove Healthcare consists of 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes in Barron, Bloomer, Chippewa Falls, Eau Claire, Fennimore, Lodi, Osseo, Rice Lake, Spooner, St. Croix Falls, and Superior. Requirements

  • Professional health care experience required (social worker, nurse CMA, CNA, pharmacy tech, PTA, COTA, etc.)
  • Ability to multitask, juggle multiple priorities and work in a fast-paced environment
  • Proficiency in relevant software tools / applications such as Microsoft.
  • Detailed oriented with strong organizational and time management skills
  • Strong problem-solving abilities
  • Proficiency in written and verbal communications

Nice To Haves

  • Experience with EPIC/Care Anywhere, Point Click Care (CRM) is helpful.
