About The Position

The Administrative Director of Value-Based Care Strategy (Director), leads operational and strategic initiatives across the Greater Lawrence Family Health Center’s value-based care portfolio. This includes driving performance, leading program teams and ensuring alignment between clinical excellence, quality outcomes, and payer expectations. Within the context of a value-based care model and a focus on population health strategy, this role requires a blend of project leadership and tactical execution on all programmatic initiatives. In collaboration with senior leadership, the Director will ensure strategic alignment with organizational goals and regulatory compliance. The Director is a seasoned, self-directed leader with the ability to manage in a matrix model and effectively work across departments and in collaboration with functional leaders to drive initiatives and sustain results. This position is anchored at the juncture of population health strategy, payer performance, clinical transformation, and data-driven execution.

Requirements

  • Bachelor Degree in healthcare, business administration or healthcare management required
  • Minimum 5 years or greater in a healthcare setting, including primary care or other ambulatory practice.
  • Demonstrable and progressive experience in fields such as managed care, population health, or performance improvement, planning, or business management.
  • Experience in an ACO, managed Medicaid, or other value-based practice environment required.

Nice To Haves

  • Master degree in administration, healthcare management highly desirable
  • LEAN or other process improvement certification highly desirable

Responsibilities

  • Supports the Senior Vice President of Clinical Services (SVP) and the Associate Vice President of Quality and Clinical Integration (AVP) in driving new program initiatives as informed by risk stratification, risk coding opportunities, regulatory requirements, payer contracts, and data analysis.
  • In support of driving population health initiatives, contributes to strategic program planning, including ongoing program evaluation based on quality, utilization, and cost metrics across the Accountable Care Organization (ACO) and other risk contracts, including but not limited to Medicare Shared Savings Program (MSSP), United, Molina, Blue Cross.
  • In collaboration with Senior Leadership and members of the Value-Based Care Governance Committee, the Director helps to develop and execute intervention strategies to improve outcomes, cost performance and shared savings results.
  • Leads the strategic development, implementation, and performance management of home visit program, Annual Wellness Visits (AWV), chronic care management (CCM), and remote patient monitoring (RPM), ensuring alignment with value‑based care objectives, regulatory requirements, and payer contract expectations.
  • Collaborates with the Business Intelligence team to analyze data, using a variety of data tools, highlighting key insights, and creating meaningful dashboards and other data visualizations to support and drive decision making and action planning processes.
  • In collaboration with department heads and other ACO/risk contract stakeholders, directs program implementation work plans across projects and teams.
  • Closely collaborates with operations, population health services, quality, information systems, analytics, finance, legal, and compliance to ensure programs are in alignment with scope and regulatory guidelines.
