The Addiction Specialist Coordinator supports individuals with Substance Use Disorders (SUD), including opioid and methamphetamine use, through comprehensive care coordination and engagement with Medication-Assisted Treatment (MAT) services. The Coordinator strengthens connections between hospital and emergency department (ED) discharges, outpatient care, home health, nursing, psychiatry, and case management to promote continuity and stabilization. NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Requirements

  • Strong understanding of harm reduction, motivational interviewing, and trauma-informed principles.
  • Excellent interpersonal and communication skills with the ability to build rapport and trust.
  • Skill in organization, multitasking, and maintaining attention to detail in a fast-paced setting.
  • Proficiency in electronic health record systems and Microsoft Office Suite.
  • Ability to manage confidential information with integrity and professionalism.
  • Capacity to work collaboratively within a multidisciplinary team and across systems.
  • Comfort in de-escalating crises and supporting clients through behavioral health challenges.
  • At least 2 years’ experience in addiction services, case management, or healthcare coordination preferred.
  • Bachelor’s degree in human services, social work, psychology, or related field preferred or equivalent experience.
  • Previous direct experience with homeless population preferred, or previous experience in a related direct human service field working with underserved populations preferred.

Nice To Haves

  • Familiarity with MAT, harm reduction, and trauma-informed care models strongly preferred.
  • Experience working with people experiencing homelessness or behavioral health challenges highly desirable.
  • May need valid driver license and clean driving record
  • Bilingual or multilingual skills preferred.

Responsibilities

  • Coordinate comprehensive care for clients engaged in Substance Use Disorder (SUD) treatment, ensuring integration across behavioral health, medical, psychiatric, and case management services.
  • Facilitate access and continuity of care by coordinating referrals, transitions from hospitals or detox programs, and re-engagement following missed appointments or lapses in treatment.
  • Support medication-assisted treatment (MAT) by collaborating with prescribers, nurses, and care teams to promote adherence, timely follow-up, and consistent client engagement.
  • Implement and monitor Contingency Management (CM) initiatives to support recovery for individuals with stimulant and opioid use disorders, including tracking participation and documenting outcomes.
  • Provide client-centered care management focused on stabilization, recovery, housing retention, and connection to long-term supports and benefits.
  • Promote client safety and crisis stabilization through proactive engagement, crisis intervention, and coordination of emergency or behavioral health services as needed.
  • Collaborate across disciplines and with community partners to enhance coordination, address systemic barriers, and improve access to services.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR) to support care continuity and compliance with organizational and regulatory standards.
  • Contribute to a culture of quality improvement by identifying service gaps, sharing best practices, and participating in team learning and development.
  • Ensure confidentiality and uphold professional and ethical standards in all interactions and documentation.
  • Perform other job duties as assigned.
