About The Position

Acute Care practitioner provides quality clinical services to Neighborhood Health patients within an integrated mental health and community-based urgent care setting. Activities involve direct patient care, physical assessment, diagnosis, and treatment of urgent medical complaints. Practitioner will perform medical clearance examinations for short term residential and mental health crisis stabilization patients in collaboration with the Fairfax County emergency mental health team. Demonstrates dedication and loyalty to the philosophy and mission of Neighborhood Health.

Requirements

  • Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program
  • Current Advanced Practice Registered Nurse (APRN) or\Physician Assistant (PA) Licensure in Virginia
  • Current provider card in Basic Life Support (BLS)
  • Experience with information technology, electronic health records, medical records, culturally diverse populations and care of underserved populations.
  • Ability to work with mentally ill individuals in crisis in a sensitive and supportive manner.
  • Ability to collaborate with a diverse workforce of primary care, mental health, and law enforcement.
  • Ability to exercise independent clinical judgement in collaboration with supervising physician
  • Dedication to the mission of Neighborhood Health and to working with an underserved population.
  • Excellent interpersonal communication skills.

Nice To Haves

  • Prior outpatient clinical experience preferred.
  • Bilingual Spanish/English a plus.

Responsibilities

  • Provides medical care and advice to Neighborhood Health patients and patients referred by the Fairfax CSB as indicated and according to protocols and evidence-based practice; prescribes medication and treatment according to protocol, formulary and evidence-based practice.
  • Provides medical clearance evaluations for patients who are pending mental health short term crisis stabilization services, including interpretation of rapid on-site laboratory tests.
  • Performs patient history and physical assessment; utilizes laboratory and diagnostic tests for differential diagnoses, with follow up treatment as medically indicated.
  • Maintains electronic patient medical records in accordance with Neighborhood Health documentation policy.
  • Collaborates with the integrated mental health and primary health care team to provide treatment plan for patients requiring follow up care on an outpatient basis.
  • Participates in Neighborhood Health peer review and quality improvement program.
  • Participates on Health Center committees as appointed by Medical Director
  • Attends regular staff meetings and trainings as required by Neighborhood Health
  • Assist with developing, coordinating and recommending changes for the improvement of workflow in the program or section area.
  • Assists with developing and recommending changes to improve quality of patient care.
  • May be assigned to various project lead roles to advance the development of the department and support ad hoc needs.
  • Employee will be asked to perform additional duties and take on other responsibilities as dictated by events and circumstances.
