The Licensed Practical Nurse serves as a member of the healthcare team at UVA Health Northern VA Culpeper. This role involves establishing an authentic, personalized relationship with patients and their chosen support systems, and collaborating with other team members to provide care that promotes optimal health. The LPN utilizes best scientific evidence and compassion to assist patients throughout their health journey. The Licensed Practical Nurse accepts accountability for the provision of care, as part of the RN-led team, in accordance with current policies and procedures. This position provides and documents patient care under the direction and supervision of the Assistant Nurse Manager/Nurse Manager.

Requirements

  • High School Diploma required.
  • Graduate of accredited Practical Nursing Program required.
  • Completion of a nursing program as approved by the State Board of Nursing.
  • Applicable state LPN license required.
  • BLS, ACLS (LD), NRP, AWHONN Intermediate Fetal monitoring and/or NCC EFM Certification within one year (for Women Children Services).
  • CPI certification within three months of hire required (for Behavioral Health, Emergency Department, and Central Staffing and Scheduling Office (CSSO)).
  • Ability to multitask and prioritize quickly.
  • Ability to communicate verbally and in written form.
  • Basic computer knowledge.

Nice To Haves

  • 1 year of relevant experience preferred.

Responsibilities

  • Serve as a member of the healthcare team at UVA Health Northern VA Culpeper.
  • Establish an authentic personalized relationship with the patient and their chosen support system.
  • Collaborate with other team members to provide care that promotes optimal health.
  • Utilize best scientific evidence and compassion to assist the care of the patient during their health journey.
  • Accept accountability for provision of care, as part of the RN led team, in accordance with the current policies and procedures.
  • Provide and document patient care under the direction and supervision of the Assistant Nurse Manager/Nurse Manager.
  • Provide evaluation, and management of patients.
  • Participate in the collection of patient data as needed.
  • Determine nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.
  • Identify relevant resources based on necessity and availability.
  • Participate as part of the interdisciplinary care team in planning individualized care.
  • Collaborate with the registered nurse to consider ongoing modification of care plan based on patient’s needs, capabilities, and condition.
  • Demonstrate core knowledge of biomedical and clinical sciences and the application of this knowledge to patient care.
  • Identify nursing interventions and goals for the patient and review goals with the registered nurse.
  • Identify opportunities for process improvement, evidence-based practice, and research.
  • Engage in self-reflection and develop personal goals to enhance knowledge, obtain professional certification, or pursue academic degree.
  • Integrate gained knowledge into practice.
  • May be asked to perform additional duties as assigned.
