The Community Navigator will support their clients by providing information, technical assistance, and support. The Community Navigator will also develop, manage, and implement training for clients. In addition, the Community Navigator will coordinate with employers and assist during the recruitment process.

Requirements

  • Must be 18 years old or older
  • High school diploma or high school equivalency (GED)
  • Clearance of a criminal background check
  • Clearance of North Carolina Health Care Abuse Registry check
  • Valid driver's license
  • Qualified in CPR and first aid/willing to attain certification
  • Exceptional organizational and communication skills
  • Ability to make long term connections for individuals, their families, and the agency
  • Effective interpersonal and communitive skills that consistently respect participants and family members and their ability to make choices
  • Ability to develop and maintain positive relationships with community agencies

  • Demonstrating full knowledge of the NC Individual and Family Directed Service Model
  • Have good knowledge of community resources, opportunities, and valuable connections within the community
  • Skilled in facilitating and providing programs for adult learners including identifying updates in materials and instructional strategies.
  • Experience with assisting participants and family members in developing support networks that include natural (unpaid) and other community supports
  • Ability to effectively advocate for participants with intellectual and other developmental disabilities and collaborate with legal guardians, family members, service providers, professionals, and members of the community on behalf of the participant the community navigator is supporting
  • Understands the principles of self-direction services
  • Trained in: Communication, Cultural Diversity, Alternatives to Restrictive Interventions, Blood Borne Pathogens, HIPPA and Confidentiality, Client Rights and Responsibilities

Responsibilities

  • Develop, implement, and maintain training programs for employers while meeting state guidelines
  • Prepare participants in locating and accessing Non-Medicaid community supports/resources
  • Assist the employer in identifying and sustaining an effective personal support network of family, friends, and paid supports
  • Provide technical assistance with developing, implementing, and modifying a back-up plan for services, staffing for emergencies, and/or worker absences
  • Monitor trainee progress, review homework, and answer questions
  • Provide education and guidance, throughout the training the process
  • Hold virtual/phone meetings with trainees to ensure understanding
  • Assist employer with compliance with Medicaid funding
  • Assist the employer in developing effective recruiting, interviewing, hiring, scheduling, supervising and payroll practices
  • Maintain regular contact with employer and their contacts
  • Complete required contact documentation in a timely manner
  • Process and request related paperwork and manage electronic communication
  • Register and enroll trainee in trainings as appropriate
  • Issue a certificate of completion to certified trainees
  • Ability to work flexible hours to meet the needs of participants/employers
  • Maintain confidentiality regarding all communication and information related to the participant, employees and other people involved with the individual's services.
