About The Position

Assertive Community Treatment Team (ACTT) What the Team Does Our Assertive Community Treatment Team (ACTT): Provides services 24/7/365 with same-day crisis response Delivers care in clients’ homes, workplaces, and community settings (not office-based) Focuses on keeping clients out of the hospital and living independently Serves adults with schizophrenia, bipolar disorder, major depression, and co-occurring substance use disorders The goal is concrete: reduce psychiatric hospitalizations and emergency room visits while improving housing stability, employment participation, and daily functioning. What You Will Own Clinical Leadership Supervise psychiatrists, nurses, therapists, case managers, substance use specialists, and peer support staff (team size: 10–12). Lead daily team meetings (5x per week) and coordinate individualized treatment plans. Oversee crisis response and risk management for a shared caseload of 80–100 clients. Program Performance Maintain ACT model fidelity and compliance with NC Medicaid and state regulations. Track outcomes: hospital readmission rates, housing stability, medication adherence, and service engagement. Review documentation weekly and complete required reporting on schedule. Community Presence Build referral pipelines with local hospitals, courts, shelters, and primary care providers. Meet families in homes and community settings to reduce relapse triggers and improve support systems. This role is 70% field + team leadership / 30% documentation + oversight . What You Bring Master’s degree in Social Work, Counseling, Psychology, or related field Active NC licensure (LCSW, LCMHC, LMFT, or LCAS) 3–5 years leading clinical staff in community mental health, crisis services, or substance use treatment Experience with ACT, CST, ICM, or other intensive community-based models Comfort managing high-acuity clients and supervising crisis interventions If you have led a team responsible for reducing hospitalizations or managing shared caseloads above 60 clients, this role aligns closely. Requirements

  • Master’s degree in Social Work, Counseling, Psychology, or related field
  • Active NC licensure (LCSW, LCMHC, LMFT, or LCAS)
  • 3–5 years leading clinical staff in community mental health, crisis services, or substance use treatment
  • Experience with ACT, CST, ICM, or other intensive community-based models
