About The Position

You will serve adults with: Severe mental illness Co-occurring substance use disorders You will work inside an Assertive Community Treatment Team (ACTT) model — services delivered in homes and community settings, not an office-only role. Since 2006, Pathways to Life has delivered community-based behavioral health services across North Carolina. Services focus on integrated mental health and substance use treatment delivered in real-world settings. This role directly impacts adults who are at risk of hospitalization, incarceration, or homelessness. If you hold a CCS, CCAS, or CSAC credential and want to practice integrated dual-diagnosis treatment in the community rather than an office, apply now.

Requirements

  • You already meet North Carolina Qualified Professional standards under 10A NCAC 27.G.0104
  • You hold one of the following active credentials: CCS CCAS CSAC
  • You have direct experience with: Dual diagnosis treatment Crisis intervention 24/7 on-call rotation
  • You are comfortable entering homes, de-escalating tense situations, and collaborating with a 6–10 person multidisciplinary team
  • Drive daily to client locations
  • Lift up to 25 lbs
  • Work in homes, shelters, hospitals, and office settings
  • Participate in evening/weekend/holiday on-call rotation
  • You may encounter high-risk or volatile situations.
  • Crisis response training is required.

Responsibilities

  • Complete substance use and co-occurring disorder assessments
  • Build individualized treatment plans with the ACTT team
  • Deliver 1:1 substance use counseling using: Motivational Interviewing Cognitive Behavioral Therapy Relapse prevention strategies
  • Provide crisis response (rotating 24/7 coverage)
  • Deliver services in homes, shelters, hospitals, and community settings
  • Document services to meet Medicaid standards
  • Facilitate person-centered planning meetings
  • Support anger management and behavioral stabilization
  • Coordinate care with psychiatry, nursing, employment specialists, and peer support
