Somerfield Health Center at The Heritage at Brentwood is currently seeking a part-time Activity Partner to assist with overall planning implementation of various recreational activities designed to meet the needs of our health center senior residents. Requirements

HS diploma or GED
Proficient computer skills, including Microsoft Word

Nice To Haves

Experience working with geriatric population preferred
Experience in social or recreational program for licensed health care setting preferred

Responsibilities

Activity Partners serve as therapeutic recreations assistants under the Activities Director for skilled nursing facility and assisted living facility residents
Review resident activity needs and plan recreation programs based on individual needs and resident interest, including music, art, discussion groups, re-motivation therapy, sensory stimulation, and other special entertainment
Directs movement of residents to and from recreational areas
Prepares and maintains daily participation logs and schedules in accordance with state and federal regulations
Maintains accurate documentation of residents’ progress and responses to programs
