The Activities Coordinator is a member of the PACE Interdisciplinary Team (IDT), actively participating in care coordination activities, including assessments and care planning. This role involves designing, implementing, and overseeing engaging activities tailored to the physical, cognitive, social, spiritual, and social well-being of PACE participants. The coordinator will utilize various techniques such as arts and crafts, animals, sports, games, dance and movement, music, and community outings. The program includes a planned schedule of social and purposeful independent or group activities, promoting an engaging environment. Remote technology may be used based on participant comfort, electronic devices, and interests. The Activities Coordinator must adhere to PACE program guidelines, regulatory requirements, and the organization's policies and best practices. This position is under the direct supervision of the PACE Care Center Supervisor at PACE centers.

Requirements

  • Minimum of a High School diploma or equivalent.
  • Completion of 36 hours of Activity Leader (previously “Activity Director”) course, designed specifically for this position and approved by the State of California, is required within 30 days of hire.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Nice To Haves

  • A bachelor’s degree.
  • Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population.
  • Experience with dementia care.
  • Bilingual English/Spanish/Mandarin/Cantonese, depending on location.
  • Experience in providing virtual activities.

Responsibilities

  • Actively participating in all care coordination activities, including Assessments and Care Planning.
  • Designing, implementing, and overseeing a variety of engaging activities tailored to the physical, cognitive, social, spiritual, and social well-being of the PACE participants.
  • Utilizing a variety of techniques, including but not limited to arts and crafts, animals, sports, games, dance and movement, music, and community outings.
  • Providing a planned schedule of social and other purposeful independent or group activities and promoting an engaging environment for the participants.
  • Using remote technology depending on the participants’ comfort level, electronic devices, and interests.
  • Adhering to PACE program guidelines and regulatory requirements, adhering to the PACE organization's policies and procedures, and best practices.
