LIFE COORDINATED is an integrated care company and PACE Program (Program of All Inclusive Care for the Elderly) and this is the way we operate. An Activities Coordinator is integral to delivering comprehensive and integrated care. We are seeking an Activities Coordinator to join our team that wants to be an integral part of an interdisciplinary care team (IDT) changing lives every day. PACE allows us to invest in the right things to address root cause and environmental factors, not just symptoms and resulting medical care. The Activities Coordinator is responsible for the planning, supervision, and implementation of therapeutic and recreational programs for the elderly, as part of a multi-disciplinary team providing all-inclusive care. The Activities Coordinator builds a sense of community and warm social atmosphere for the participants, evaluates their needs, and develops groups and individual programming to meet those needs.

Requirements

  • At least one year of professional work with an elderly or frail population.
  • Minimum 2 years’ experience in a social or activity/recreational therapy program, providing and coordinating services for a frail or elderly population within the past years.
  • Interest and ability to work within a multi-disciplinary team.
  • Demonstrated understanding of psychosocial problems of aging.
  • The ability to effectively work within an interdisciplinary team
  • The ability to work effectively with culturally, economically, and educationally diverse populations
  • The ability to form positive interpersonal relationships with a wide range of staff and clients.

Responsibilities

  • Knows and adheres to the philosophy and goals of (PACE Program).
  • Maintains a quiet and safe environment for clients, visitors, and staff.
  • Keeps confidentiality of client records, reports, and discussions.
  • Participates in formulation and maintenance of (PACE Program) policies and procedures.
  • Participates in (PACE Program) Committees as requested by the Clinical Services Director.
  • Actively participates in Interdisciplinary Team Meetings (IDT).
  • Attends and participates in scheduled staff meetings and client care meetings as requested.
  • Advises the Clinical Services Director in ways and means to establish better accountability of (PACE Program) services to clients and referral sources.
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
  • Assists in orientation and supervision of staff/volunteers.
  • Utilizes supplies and equipment economically.
  • Maintains timely and quality documentation of clinical services provided, reviewing medical record for continuity and completeness, and to meet compliance and regulatory requirements.
  • Supports Quality Management Program through participation in quality studies, provision of recommendations for improvement, and active participation in performance improvement actions.
  • Pursues professional growth through the continuing education process.
  • Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Clinical Services Director as appropriate; responds in a positive manner to constructive criticism; serves as a role model for students and staff members.
  • Optionally holds membership in professional organizations.
  • Performs other duties as required or requested in a positive and helpful manner to ensure a smooth-running work environment.
  • Conducts an initial assessment and scheduled reassessment(s) of all participants to evaluate educational, emotional, and vocational needs.
  • Develops specific therapeutic programs to include recreational and social activities.
  • Plans, supervises, and implements relevant recreational programs to meet the needs of the participants, per the treatment plans developed by the IDT.
  • Builds within the PACE facilities a sense of community and warm social atmosphere in which participants can identify a support system.
  • Organizes and directs individual and group activities, reality orientation, exercises, discussions, clubs, field trip/outings, arts and crafts, music appreciation, etc.
  • Coordinates with various community resources to add diversity and enrich the recreational program.
  • Establishes and maintains cooperative working relationships with other program staff, contact agencies, and outside organizations. Serves as a staff resource.
  • Communicates effectively with hospital departments to minimize hospital lengths of stay as appropriate and allow for a smooth transition for the client as he/she moves from the hospital to alternative levels of care.
  • Participates in quality management program activities.
