About The Position

This position of moderate difficulty is responsible for providing a variety of case management and outreach interventions to consumers in their natural environment. Serves as advocate in assisting consumers in accessing community resources, teaching and modeling self-help and coping skills.

Requirements

  • A Bachelor’s degree in social work, psychology, counseling, or a related field and two years of full-time work experience in the field of mental health.
  • Any combination of an associate degree with two years of experience in community mental health outreach/case management or four years experience in community mental health outreach/case management may be substituted for a bachelor’s degree.
  • A Vocational Specialist is a person with a minimum of one-year verifiable training and/or experience in vocational counseling.
  • Must have excellent assessment and crisis management skills.
  • Candidate will work closely with metro area hospitals, city/county jails and courts.
  • Must be able to negotiate system boundaries.

Responsibilities

  • Attend and participate inTreatment Team
  • Assist CSR with Job Readiness Skills, Case Management, and Skill building activities
  • Maintain a case load as assigned by Team Lead
  • Meet the minimum unit count of 360 units per month
  • Collaborate & Coordinate with Supported Employment Agencies
  • Develop Job Sites
  • Monitor consumers on job sites
  • Ensure all Failed Activities are completed within 24hrs
  • Participate in ACT Fidelity Review
  • Participate in ASO Audit
  • Participate in CARF Review
