About The Position

New Horizon Counseling Center is committed to empowering individuals living with serious mental illness to lead meaningful, independent lives. Through compassionate, community-based care, we provide comprehensive behavioral health services that meet clients where they are. We are seeking a dedicated and compassionate Case Manager to join our Assertive Community Treatment (ACT) Team. The ACT Team Case Manager plays a critical role in delivering intensive, community-based mental health services to individuals with severe and persistent mental illness. This position involves working collaboratively within a multidisciplinary team to support clients in achieving stability, independence, and improved quality of life.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s preferred)
  • Minimum of 6 months experience working with individuals with serious mental illness
  • Knowledge of community resources and recovery-oriented practices
  • Strong communication, organizational, and problem-solving skills
  • Ability to work independently and as part of a multidisciplinary team

Nice To Haves

  • Experience with ACT or other intensive community-based programs
  • Bilingual skills are a plus
  • Familiarity with electronic health records (EHR) systems

Responsibilities

  • Provide direct, community-based services to clients, including home visits and outreach
  • Develop, implement, and monitor individualized treatment plans in collaboration with clients and the ACT team
  • Assist clients with daily living skills, housing stability, employment, and social integration
  • Coordinate care with internal team members and external providers, including healthcare, social services, and family supports
  • Monitor client progress and document services in accordance with agency and regulatory standards
  • Provide case management using motivational interviewing to help clients to utilize their strengths and abilities to address their problem
  • Address risk factors such as substance use, poor use of leisure time, and self-doubt by applying problem-solving services
  • Promote client benefits and income support by escorting consumers to appointments for Medicaid, food stamps, SSI/D, HRA, child support etc.
  • Provide crisis intervention and support as needed
  • Participate in team clinical meetings, case conferences, and on-call rotation as required
  • Other duties as assigned
