The ACT Rehabilitation Specialist plays an important role on the Assertive Community Treatment (ACT) team by helping members obtain and maintain involvement in employment and/or community-based activities. Works as member of a clinical team to assess member needs, participates in the development of individual service plans, and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the ACT team’s specialist for rehabilitation services by collaborating with other team members in the assessing and delivery of rehabilitation skills training and meaningful community activity as specified in the individual consumer’s service plan.
