About The Position

The Alliance Acute Outpatient Program (AOP) provides mental health services for clients with acute presenting concerns; substance use disorders and have frequent crisis involvement. The Assertive Community Treatment (ACT) team provides non-traditional community based mental health treatment to promote recovery and community integration to individuals who may be experiencing serious mental illnesses, substance use disorders, and other psychosocial stressors. The Assertive Community Treatment Team Employment Specialist will be responsible for providing work-related services through therapy and/or care coordination services for clients on the ACT team.

Requirements

  • High School Diploma or GED is required
  • A past or current consumer of mental health services. Must possess the ability, knowledge, and personal insight to maintain mental stability.
  • Peer Specialist Certification if not already certified.
  • The person must be able to demonstrate self-motivation, and excellent communication skills, and ability to work well with others.
  • The person must be a good listener, possess the ability to be supportive, and be able to help others to achieve a feeling of well-being.
  • This individual will be part of two multi-disciplinary teams will require collaboration and consultation with team members of various roles as well as ability to maintain positive relationships community stakeholders and must demonstrate an ability to use mature judgment in problem solving.

Responsibilities

  • Provide peer support services in a community setting.
  • Facilitate psychosocial rehab groups.
  • Ensure client’s psychosocial needs are documented in the treatment plan objectives and interventions (under appropriate related diagnoses).
  • Understand and monitor community connectedness outcomes for clients served.
  • Share personal, practical experience to benefit the team and clients.
  • Provide expertise about symptom management and the recovery process.
  • Promote a team culture that maximizes client choice and self-determination.
  • Provide peer counseling to ACT clients and families.
  • Carry out other rehabilitation and support functions.
  • Work according to fidelity with the ACT model.
  • Demonstrate a trauma-informed, person-centered approach to treatment.
  • Involving family members and natural supports as available/appropriate in treatment process.
  • Maintain positive and collaborative community and peer relationships.
  • Must be willing to provide transportation in personal vehicle.
  • Achieve agency standard productivity goals by providing designated number of monthly contacts as a high-quality service.
  • Maintain high quality, collaborative documentation on all visits provided.
  • Take rotation in the on-call process for the ACT team to ensure 24/7 coverage.
  • Other duties as assigned.
