About The Position

A healthcare plan is seeking an Account Manager to manage and maintain existing referral relationships within the Managed Long-Term Care (MLTC) and MAP space. This role focuses on relationship management — not new business development — working closely with home care agencies and community partners to maintain strong referral pipelines. This is a highly field-based role requiring strong people skills, consistent follow-up, and relationship-building within the community.

Responsibilities

  • Maintain and strengthen existing referral relationships (primarily home care agencies)
  • Manage a strong existing book of accounts and ensure ongoing engagement
  • Conduct regular visits to home care agencies and community partners
  • Educate referral sources on plan benefits and enrollment process
  • Ensure qualified patients enroll in appropriate healthcare plans
  • Track outreach activities and maintain thorough follow-ups
  • Support membership growth through relationship management
  • Coordinate with internal teams on referrals and member needs
