As an Access Nurse, your work will focus on coordinating patient flow. In this position, you will facilitate hospital transfers and direct admissions, determine daily bed resources and oversee bed management logistics for the health system. This position is vital in gathering accurate data to enhance efficiency and promote patient safety and need prioritization. This is a remote position. A caregiver in this role works between 7:00pm and 7:00am.

Requirements

Applicants must reside in Florida
Graduate of an accredited school of Professional Nursing
Current state license as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
Three years of recent progressive clinical experience in an acute care setting

Nice To Haves

Bachelor’s of Science in Nursing (BSN)
Two years of supervisory experience in a healthcare setting
Three years of RN experience
Computer proficiency in Microsoft applications, bed management databases and Epic

Responsibilities

Serve as the primary point of contact for telephone patients with a variety of health or illness questions.
Utilize effective and professional communication, clinical knowledge and nursing assessment skills to quickly triage patient status.
Provide intake, assessment and triage for internal and external hospital transfers and direct admissions on request in accordance with standard operating procedures.
Elicit information from referring providers, review patient clinical information and assess patient status for appropriate placement.
Collaborate with health care providers and hospital leadership to optimize patient outcomes utilizing clinical expertise and incorporating nursing and management experience with critical thinking skills and business intelligence tools to achieve optimal outcomes both clinically and financially for the patients and the organization.
Apply solid understanding of the complexities of the patient status and the process and capabilities of each system hospital to seamlessly facilitate and achieve enterprise bed placement.
Provide oversight of all bed management functions.
Function at any point in the patient flow process designated to ensure the safe and efficient placement of patients and effective utilization of resources.
Demonstrate cost effective management of enterprise bed resources to enable the organization to provide quality care to patients.
