About The Position
The ABI Case Manager at Cooperative Production plays a crucial role in enhancing the lives of individuals with Acquired Brain Injury (ABI), Traumatic Brain Injury (TBI), and various disabilities. This position involves developing, managing, and implementing a comprehensive array of services tailored to meet the unique needs of these individuals. The ABI Case Manager will work closely with clients to provide case management, transportation, and support in developing social relationships, as well as training in therapeutic, recreational, and personal living skills. The focus is on delivering services that align with the agency's mission of promoting quality of life and dignity for all clients. In this role, the ABI Case Manager will be responsible for ensuring that services are meaningful and delivered in a dignified manner, enhancing the self-esteem and status of individuals as adults. This includes developing and implementing activity schedules based on individual needs and preferences, assessing participant satisfaction, and applying clients' strengths and interests to foster community participation. The ABI Case Manager will also supervise assigned staff, ensuring adherence to agency policies and providing necessary training and feedback. The position requires collaboration with other staff members to coordinate services across various program components, as well as the development and monitoring of Individual Support Plans (ISPs). The ABI Case Manager will assist in ensuring compliance with state and federal regulations and will be involved in public relations efforts to promote the agency's mission. Additionally, the role includes responsibilities for data collection and documentation, ensuring timely communication of relevant information to both internal and external stakeholders.
