About The Position

Under the supervision of the AB109 Program Coordinator, the AB109 Case Manager is responsible for providing re-entry services to clients that are referred by the San Joaquin County Probation Department and initiate proper referrals to resources as needed.

  • Must complete and pass criminal and fingerprint background check.

  • A Bachelor’s in Criminal Justice or related field with 1-3 years of experience working with at-risk population is highly preferred; An Associate’s Degree in Criminal Justice or related field with 2-4 years of experience working with at-risk population is highly preferred; A High School Diploma or GED Equivalency and 3-5 years of experience working directly with AB109 population is required.
  • Valid Driver’s License
  • Up to date automobile insurance
  • Successful completion of Sexual Harassment Prevention Training
  • Successful completion of Workplace Violence Prevention Training
  • Successful completion of HIPPA Training

Nice To Haves

  • Bilingual in Spanish is highly preferred.

Responsibilities

  • Adheres to El Concilio California policies and procedures per the agency employee manual.
  • Contact new referrals for initial contact and schedule orientation no later than two days after receiving referral.
  • Required to complete intake no later than 30 days after receiving referrals.
  • Maintain accurate and thorough documentation of program participants progress on a daily basis per probation department standards.
  • Must conduct weekly contact with program participants via phone, email and in-person meetings.
  • Required to do home visits and follow AB109 Safety Protocols.
  • Assist program participants with the completion of any formal documentation that will provide access to resources, such as housing, education, transportation, clothing, food, employment, etc.
  • Initiate referrals and advocate for program participants to receive adequate information to outside resources.
  • Required to manage a case load of 30-60 program participants.
  • Must conduct outreach to receive new program participants at local agency events.
  • Attend meetings with probation and county officials based on program related purposes.
  • Required to attend AB109 and/or El Concilio California trainings throughout the weekday and weekends agency requirements.
  • Assist AB109 Program Coordinator and Lead AB109 Case Manager with the development and training of new AB109 Case Manager(s).
  • Provide coverage at the Front Desk.
  • Maintain professional relationship with all program participants and El Concilio California personnel.
  • Required to work El Concilio California special events and meetings.
  • Other duties as assigned by immediate supervisor and administration.
