About The Position

In this role as a 245D Service Coordinator, you will oversee and coordinate services for individuals enrolled in a 245D program. You will ensure the delivery of high-quality, person-centered support by implementing care plans in accordance with federal and state regulations, as well as organizational policies. This position is part of a broader fiscal services team and offers significant autonomy in managing your assigned caseload.

Requirements

  • Two-year degree in Human Services or related field and a minimum four years of relevant work experience; a four-year degree in human services or related field and a minimum of two years of relevant work experience preferred.
  • A minimum of two years 245D experience.
  • 20 – 35% travel as required to interact with persons, their families, county personnel and external training.
  • Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff. Excellent verbal, written, and phone communication skills.
  • Ability to manage multiple priorities and take initiative with work objectives.
  • Strong analytical and financial acuity skills.
  • Ability to work independently; organize work consistently and meet deadlines.
  • Demonstrated skills in problem-solving with an optimistic and compassionate nature.
  • Ability to collaborate with co-workers across departments and teams.
  • Travel in personal vehicle in the greater metro area is required.
  • Candidate must complete and pass a background study, pre-employment drug screen; have a valid driver’s license free of violation; proof of motor vehicle insurance and reliable transportation for work purposes.
  • Candidate must complete and pass a pre-employment background study; have a clean driving record & valid driver’s license, a physical and drug screen; and have access to reliable transportation for work purposes.

Nice To Haves

  • Thorough understanding of disability services and funding sources available in Minnesota highly preferred.
  • Case management experience in licensed services is highly preferred.

Responsibilities

  • Overseeing and managing all 245D-related documentation and paperwork.
  • Ensuring direct care employees receive thorough training, scheduling, continuous assistance, and evaluation.
  • Conducting thorough client admissions, intakes, and evaluations.
  • Creating and carrying out individualized service plans based on the requirements of each client.
  • Managing a caseload of clients with professional customer service, case management, and basic calculations and budgeting skills.
  • Communicating verbally, in writing and in person with the individuals we support, their support teams, county case manager, and referral sources.
  • Reviewing progress and making any necessary revisions to service plans during regular meetings with clients, families, and care team.
  • Managing the individual’s fiscal budgets using technical tools, spreadsheets, and utilizing basic calculations and budgeting abilities.
  • Maintaining a working knowledge and understanding of Minnesota waiver programs.
  • Keeping all records and documentation current, accurate, and compliant.
