In 1935, Louise Whitbeck Fraser opened a school in her home for people with disabilities. Fraser is Minnesota’s leader in autism, mental health, and disability services, and a Certified Community Behavioral Health Clinic (CCBHC). The organization is a nonprofit committed to integrated community behavioral healthcare, improving quality, accessibility, and coordination of care, leading with compassion, innovating with purpose, and fighting for inclusion.Fraser is seeking a Program Coordinator to join its Community Living team. This role involves providing supervision and leadership to ensure continuous quality improvement in active treatment service development and implementation, while complying with all regulations, Fraser's vision, policies, and budgetary parameters. The position requires 245D qualification.The Community Living team has two divisions:Supervised Living: Over 20 group homes, each with 4 residents requiring 24/7 supervision and assistance with hygiene, housekeeping, activities/appointments, daily hands-on support, and medication passing.Supportive Living: 5 apartment settings where residents are more independent, do not require 24/7 supervision, and need verbal prompting. The Program Coordinator oversees one apartment complex.

Requirements

  • 2 years of professional experience working with individuals with mental health needs
  • Valid driver's license
  • Must be 245D qualified in one of the following ways: HS Diploma or Equivalent AND four years of full-time work experience providing direct care services to persons with disabilities
  • Must be 245D qualified in one of the following ways: BA/BS in Human Services or related field AND 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities
  • Must be 245D qualified in one of the following ways: Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities

Responsibilities

  • Coordinate intakes and discharges in adherence with policies and procedures.
  • Remain abreast of and follow all regulations and best practices in the delivery of services.
  • Communicate staffing needs to Human Resources/Staffing Coordinator.
  • Submit all billing to Accounting Department in accordance with the schedule.
  • Monitor utilization of service hours to avoid over or under utilization.
  • Coordinate services with other professionals, such as physicians, mental health professionals, and IDT/special education teams, as applicable.
  • Carry and respond to all incoming calls on the Community Living emergency cell phone in accordance with the emergency cell phone and incident response procedures.
  • Provide culturally responsive services using interpreter services when requested.
  • Provide job training, direction, and supervision as required by regulations, Fraser policies and procedures or as needed.
  • Ensure individual billing forms and time clock exception forms are submitted to accounting in accordance with the payroll schedule.
  • Provide leadership and guidance to employees supervised, including consultation for problem solving and conflict resolution.
  • Develop and implement training materials for employees supervised as needed or requested.
  • Promote professional growth and job satisfaction of staff by motivating, developing, and coaching.
  • Maintain documentation as outlined in the regulations and in accordance with Fraser’s policies and procedures.
  • Document all Records of Conversation and coaching sessions with employees supervised.
  • Maintain all records in accordance with HIPAA requirements.
  • Regular and timely attendance.
