Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime The YV LifeSet Specialist role is to support YAs become self-sufficient by assisting in locating and learning how to use community resources such as housing, employment and education.

Requirements

  • Bachelor's degree in human/social service-related field (including education, psychology, social work, or a related subject area)
  • At least 2 years of relevant experience in child welfare, social services, or related field
  • Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills
  • Ability to work independently and as part of a team to meet deadlines, & prioritize multiple tasks efficiently
  • Ability to work with a diverse population and work from a strength-based, trauma-informed, youth development lens
  • Strong crisis management abilities to handle challenging YAs when incidents arise
  • Ability to receive and execute feedback to professionally grow and/or improve and be flexible with programmatic needs and changes
  • Must be able to travel to meet YA in their homes, in the community and as needed
  • Demonstrated strong commitment to safety.

Responsibilities

  • Engage YA w/ a background in Child Welfare by building & maintaining a trusting relationship with a caseload of 8-10 YAs
  • Conduct weekly sessions to provide support & service planning with YAs in regard to obtaining and maintaining housing, obtaining and maintaining employment, education, & parenting
  • Conduct monthly assessments by developing monthly service plans to address treatment planning with YAs
  • Assisting YAs with connecting to supports as well as MH & medical services to address ongoing needs to enhance well-being & functioning
  • The ability to establish and maintain collaborative working relationships with colleagues & stakeholders in the community
  • Attend weekly coaching, consultation & group supervision with LPE/SUP to receive appropriate feedback on next steps with planning for each YA in the program
  • Attend agency/ACS/LifeSet Booster training ongoing to support with professional development
  • Complete timely and accurate documentation of YAs progress as it relates to TX Plans, Why What Assessments, PEAs, Screeners, & Session Notes in CONNXs, & Evolv database systems
