The Youth Tracker is a vital role within our organization, responsible for maintaining daily contact with youth assigned to their caseload. This position requires a flexible work schedule, which may include evenings, weekends, and holidays, to ensure that the needs of the youth are met effectively. The Youth Tracker will engage in face-to-face interactions at various locations, managing behaviors through structured programs and fostering a helping relationship that emphasizes accountability among the youth. In addition to daily contacts, the Youth Tracker is tasked with completing necessary documentation accurately and promptly, including discharge information for the youth. They will also be responsible for entering data into the Department of Juvenile Justice's Juvenile Tracking System (JTS) and the Southwest Key Programs database, Efforts to Outcome (ETO). Familiarity with each youth's case file is essential, as is the ability to respond to crises or incidents involving the youth, ensuring that the Program Director or designee is notified immediately in such cases. Regular and punctual attendance is crucial for this role, as is the ability to assist in coordinating youth activities and providing advocacy in various areas such as health, education, and legal matters. The Youth Tracker will also play a role in training new staff and may be called upon to provide staff relief as needed. Crisis intervention skills and the ability to administer first aid are important components of this position, as is the capacity to adapt to changing circumstances and handle additional tasks as assigned.
