About The Position

We are seeking individuals who are passionate about making a positive impact on the lives of adolescents. Join our team as a Direct Support Professional | Youth Support Worker and together, we will continue providing a safe, supervised and structured residential setting for at-risk adolescents. About Hope Homes – Holyoke Hope Homes – Holyoke House is dedicated to providing a therapeutic environment that offers individualized care and support to residents aged 14–21. Our professionals play a crucial role in guiding adolescents through unique challenges, empowering them to reach their full potential. We believe in fostering a supportive and inclusive community. The Ideal Youth Support Worker | Direct Support Professional Possesses strong communication and interpersonal skills, capable of building meaningful relationships with residents, families, and team members. Has knowledge and understanding of the unique needs and challenges faced by individuals with autism, intellectual, and developmental disabilities. Can assist with daily living activities and implement behavior management strategies. Demonstrates composure in challenging situations, exhibiting patience and understanding.

Requirements

  • A high school or high school equivalency diploma and one year of experience working with individuals with I/DD in a group setting; OR
  • An associate or bachelor’s degree from an accredited college or university in a field that is unrelated to social work or psychology and six months’ experience working with individuals with I/DD in a group setting; OR
  • An associate or bachelor’s degree from an accredited college or university in social work, psychology or a related field
  • Must be 21 years of age or older
  • Possess a valid driver's license
  • Physical & Drug Screening, Criminal Record History, FBI, and Child Abuse History Clearances

Responsibilities

  • Provide direct care and support to up to five residents, including supervision, meal preparation, and transportation to appointments and activities.
  • Assist in developing and implementing individualized treatment plans for each resident.
  • Collaborate with team members to ensure consistent and effective delivery of therapeutic care.
  • Participate in staff meetings, training sessions, and other career development activities.
  • Maintain accurate and complete resident records, including progress notes and incident reports.
