Job Title: Facilitator/Youth & Family Care Coordinator Division/Program: Star View Teammates Starting Compensation: 24.56 - 26.37 USD Per Hour Working Location: Los Angeles, CA Working Hours/Shift: Mon - Fri: 8 AM - 5 PM How you will make a difference: This position has primary responsibility for coordinating services across various programs, including engagement, assessment, Child and Family Team (CFT) formation and implementation, and development of Safety Crises Plan and Strengths Summary. Facilitators provide a broad range of case management services meant to stabilize and reduce involuntary hospitalization and/or placement disruption. When indicated as a need by the youth/adult/family, the Facilitator may provide mental health services such as, individual rehabilitation, collateral intervention, and coaching. He/she acts as a liaison with external agencies and convenes the Child and Family Team. Facilitators take the lead to ensure the team consists of family members, County representatives, the client, and others selected by the family. The Facilitator works to ensure all team members are active participants in the CFT Process. He/She works with family to generate Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs. Collaborates with team members to ensure adherence to program process and principles, as well as, TEAMMATES philosophy of “Whatever It Takes.” Division/Program Overview: Community-Based and Family Centered programs. TEAMMATES offers Wraparound, Intensive Field Capable Clinical Services (IFCCS) and IFCCS Outreach Triage Team and Child Outreach Triage Team services that cover a full range of treatment and support. 24 hours of referral receipt, and services are delivered countywide by the same team 24-hour Crisis Intervention SED Children and Adolescent driven program. Client and Family driven program with mutually agreed by service provider and family. Contract with DMH and Child Protected Services and Probation. Placement within the grade is determined based on experience, internal equity, and other factors permitted by law. Requirements

  • Facilitator II: Master's degree in Mental Health, Human Services, closely related Field, Facilitator I: Bachelor's degree in Mental Health, Human Services, or closely related Field.
  • Facilitator II: Master’s degree: One (1) year experience in mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high risk children and families.
  • Facilitator I: Bachelor’s degree: Two (2) years’ experience in mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high risk children and families
  • Valid California Driver's License
