About The Position

The position involves providing support to children/youth through the DCS program as they develop/maintain skills that enhance the quality of their lives in the community. The role includes supporting the child/youth with personal care and daily routines, ensuring they achieve outcomes from their Plan(s) of Care, and practicing Support Solutions' philosophy and values in interactions with the child/youth and community members. Responsibilities also include safe transport to events, school, and medical appointments, ensuring proper use of resources, cooking meals, and documenting progress towards meeting outcomes. The employee will also participate in meetings and assessments as needed, and provide thoughtful decision-making support for the child/youth's health, safety, and emotional growth.

Requirements

  • Must be 18 years of age and able to read and write effectively.
  • Must have a HS Diploma or GED.
  • Must have a valid driver's license and maintain an acceptable driving record.
  • Must meet state and local requirements regarding background checks.
  • Must be able to perform heavy lifting in assistance of service recipients.
  • Must have some computer knowledge.

Responsibilities

  • Support the child/youth with personal care and daily routines.
  • Help the child/youth become integrated with the community.
  • Ensure safe transport to events, school, and medical appointments.
  • Attend all training meetings assigned by Support Solutions or DCS.
  • Cook and provide meals based on scheduled menus.
  • Perform daily activities or teach the child/youth to do so.
  • Utilize electronic records system including completing daily shift notes.
  • Encourage customer satisfaction through regular contact with child/youth, families, DCS, and external stakeholders.
  • Oversee storage and administration of medications.
  • Participate in CFTM/IEP meetings as needed.
  • Exhibit behaviors consistent with the vision and values of SS.
  • Practice safe work habits to maintain a safe work environment.
