About The Position

The Youth Community Health Worker (CHW) plays a crucial role in assisting youth and their caregivers in accessing essential community resources to address challenges such as unemployment, food scarcity, and referrals to welfare and housing assistance agencies. This position involves collaboration with various stakeholders to support the goals of Youth Empowerment Services and ensure that participants receive the necessary support for their health and well-being.

Requirements

  • 2 years of demonstrated experience working with both youth and adults within a social agency or adolescent department of a medical center, specifically with youth having substance use disorder (SUD) issues.
  • Ability to establish rapport with adolescents, parents, and extra-agency representatives.
  • Ability to attend community meetings, take notes, and present with decorum.
  • Completion of a 3-Hour Training of the Trainer (TOT) currently offered by NYCDOHMH.
  • Working knowledge of East New York, Brownsville, and surrounding communities.
  • Practical understanding of technology, local and city-wide resources, and the welfare system.
  • Ability to prioritize and delegate multiple tasks and projects to ensure timely completion.
  • Proficient in Microsoft Office and utilizing Electronic Health Records.
  • Ability to establish and maintain effective and collaborative working relationships, demonstrating compassion, respect, courtesy, and tact when interacting with others.
  • Active listening skills to elicit needed information and communicate effectively.

Nice To Haves

  • Bilingual in English and Spanish preferred.
  • Possession of an active CPR certification.

Responsibilities

  • Work collaboratively with the Youth Empowerment Counselor and Addictions Program Manager to support Youth Empowerment Services goals.
  • Assist youth and caregivers with necessary resource referrals for available community and NYC resources, including attending community focus meetings.
  • Call and text clients to facilitate timely attendance in medical and behavioral health appointments.
  • Build rapport and communication with behavioral health, medication for opioid use disorder (MOUD), and adolescent patients to facilitate their treatment adherence.
  • Document all services rendered for patients in the electronic medical record accurately and concisely.
