Guide and mentor youth in program to becoming adults, while maintaining professional boundaries and protecting client confidentiality. Assist teens in meeting the goals set forth in their Individual Service Plans (ISPs), Conferencing and Teaming meetings, Permanency Plans, and Treatment Plans. Delivery of Life Skill Lessons to youth of the program Communicate effectively through Incident reporting, emails, logbooks, verbal reports and scheduling appointments and meetings Administer and document medication to youth Provide transportation of clients as needed for meetings, court, outings, activities, shopping, and appointments. Facilitate and Monitor group activities on and off site. Maintain safety within the program by completing room checks and safety checks throughout the building and document maintenance issues. Professionally represent the agency in court, team meetings, and other official settings.
