About The Position

The primary role of this position is to ensure high-quality implementation of milieu interventions, case management, and discharge processes that reflect organizational standards of excellence, compassion, and accountability. The Youth Care Coordinator supports whole-person care, trauma-informed practice, youth dignity, professional integrity, and measurable program outcomes while contributing to a safe, collaborative, and supportive work culture. This position includes both administrative responsibilities and direct floor support, requiring flexibility, teamwork, and a commitment to consistent collaboration.

Requirements

A bachelor's degree in human services or related field preferred (e.g. Social Work, Psychology, etc.).
Minimum 2 years of experience working with young people with mental, emotional, or behavioral needs.
MN Statute 245I Mental Health Practitioner qualification.
Must be 21 years of age or older, have a valid Driver's License and pass required background check.

Responsibilities

Intake Coordination
Participate in intake meetings with the Program Manager using a welcoming, compassionate, and youth-centered approach.
Conduct possession inventories with attention to safety, dignity, transparency, and trauma-informed handling of personal items.
Provide a warm, supportive unit orientation that promotes physical and emotional safety, cultural responsiveness, and youth empowerment.
Complete all intake documentation accurately and promptly to support strong program outcomes, care continuity, and compliance standards.
Ensure risk, safety, and wellness information is communicated clearly to the care team to promote shared accountability and collaboration.
Milieu Intervention
Collaborate with program therapists and unit supervisors to develop and refine milieu interventions that reflect trauma-informed, whole-person care principles and contribute to program efficacy.
Implement interventions consistently and model strengths-based engagement, emotional attunement, compassion, and relationship-based practice.
Provide supportive coaching, guidance, and real-time mentoring to direct care staff to build confidence, competence, and professional growth.
Care Team Coordination
Serve as the primary point of contact for families, guardians, county workers, insurance partners, and other external entities with integrity, professionalism, and clarity.
Facilitate referrals, assessments, and care coordination responsibilities in a timely, organized manner to support smooth service delivery and positive outcomes.
Provide weekly updates and follow-up communications regarding progress, concerns, strengths, or incidents, ensuring transparent and respectful communication.
Program Support
Provide coverage as needed to maintain safe staffing ratios and a stable, supportive environment, including working at leader one PM shift per week.
Assist youth using trauma-informed strategies that promote regulation, resilience, skill-building, and emotional safety.
Reinforce consistent expectations, routines, and program structures to support treatment progress and predictable living environments.
ASP On-Call Rotation
Participate in the on-call rotation to provide crisis guidance through a calm, trauma informed, supportive, and accountable lens.
Offer clear, decisive direction to staff and reinforce adherence to safety protocols, de-escalation practices, and professional standards.
All other duties as assigned.
