About The Position

The Y-NOW Mentoring Case Manager at the YMCA of Greater Louisville is responsible for supporting and advocating for youth aged 11-15 who have a parent currently or previously incarcerated. This role involves managing a caseload of mentor/youth matches, facilitating group sessions, and collaborating with community partners to ensure the well-being and progress of participants. The position emphasizes a strength-based approach to case management, focusing on building relationships and providing resources to youth, caregivers, and mentors.

Requirements

  • Bachelor's Degree preferred.
  • Minimum of 21 years of age.
  • Strong oral and written communication skills.
  • High awareness of community resources and referring agencies.
  • Availability to work some nights and weekends.
  • Ability to be out of town for several days for retreats.
  • Minimum of 2 years experience case managing volunteers/mentors or working with high-risk youth.
  • Knowledge of children with incarcerated parents and/or youth who have experienced trauma is preferred.
  • Familiarity with Trauma Informed Care and Positive Youth Development philosophies.
  • Strong multi-tasking, organization, and time management skills.
  • Strong computer skills (Outlook; Microsoft Office) and knowledge of web-based applications.
  • Valid driver's license, own vehicle, and valid car insurance.

Responsibilities

  • Uphold the mission of the YMCA and demonstrate core values of honesty, respect, and responsibility.
  • Maintain an active caseload of 30+ mentor/youth matches and caregivers.
  • Keep accurate records and statistical information for evaluating participant progress.
  • Support and foster strong mentor/mentee relationships and communication.
  • Facilitate and prepare materials for group sessions and activities.
  • Conduct regular school and home visits.
  • Maintain regular contact with caregivers and volunteer mentors.
  • Attend and facilitate meetings for/with youth at various locations.
  • Manage collaborations with community partners and school administrators for referrals and advocacy.
  • Assist with ongoing program development with the program director.
  • Develop and implement policies, procedures, and curriculum to achieve program goals.
  • Attend meetings and trainings as requested by the program director.
  • Participate in community outreach events and retreats.
  • Manage emergency situations on a 24-hour on-call basis.
  • Adhere to confidentiality and professional boundaries, following mandated abuse reporting requirements.
