About The Position

The Facilitator/Care Coordinator is responsible for leading, guiding, and collaborating with the client, family, and team members. A Facilitator/Care Coordinator is expected to exercise professional judgment and discretion in the performance of their duties. Wraparound staff must complete all training requirements per DMH contract within the first year of employment. The Facilitator/Care Coordinator reports directly to the Team Supervisor.

Requirements

  • Bachelor of Art of Science Degree in Human Services, Social Work or Psychology or a minimum of three years’ experience working with high-risk children or youth in either Child Welfare, Probation or Mental Health Systems.
  • Valid California Driver’s License, good driving record, and must be able to drive to fulfill the duties of the position.

Responsibilities

  • INTEGRATION: Ensures the successful integration of services to provide for the safety, permanency, and well-being of the Wraparound and IFCCS clients.
  • SERVICE MODEL: Understands and practices the principles and philosophy of the Shared Core Practice Model and ensures that the client’s plans are effectively and comprehensively assessed, coordinated, delivered, transitioned, and reassessed in a way that is consistent with the intent of the Model.
  • SUPERVISION: Meets regularly with supervisor concerning client and program issues; assists in development or modification of procedures to address a client’s specific needs.
  • COORDINATION: Organizes and coordinates timely interactions with the CFT members providing intensive care coordination with each assigned client and family, ensuring that all identified services are provided in a timely and appropriate manner.
  • FACILITATION: Co-facilitates the CFT meeting with the child and family through the phases of Wraparound and the CFT Model.
  • MEETINGS: Attends staff meetings, trainings and conferences as required.
  • PRODUCTIVITY: Meets all productivity requirements and completes all necessary documentation required of the position set forth by the agency Tier System and internal policies.
  • BOUNDARIES: Establishes positive, respectful and nurturing relationships with clients, families, and members of the CFT within professional boundaries and in accordance with agency policies and procedures.
  • SAFETY: Prioritizes the safety of clients by providing support to team members, direct crisis intervention, and reports critical and special incidents within 24 hours.
  • COLLABORATION: Builds consensus within the team around collaborative plans, actively supports the agenda, and ensures that family voice and choice is heard throughout the entire teaming process.
  • STRENGTH BASED: Engages the client and family to identify client and family strengths, goals, and underlying needs. Mitigates client and family reluctance to services by being patient, offers reassurance and encouragement,
  • PROFESSIONALISM: Interfaces professionally and appropriately with other agency staff and extra-agency resources.
  • RESPECT: Demonstrates respect for the intrinsic value of each person, their individual differences and culture.
  • CONFIDENTIALITY: Maintains strict confidentiality over all records and information relative to clients.
  • OTHER: Performs other duties as assigned by supervisor.
