About The Position
The Wraparound Facilitator plays a crucial role in the Conscience Community Network program by collaborating with children, families, and support systems to identify individualized needs and develop a comprehensive care plan. The Facilitator is responsible for leading Family Team Meetings, guiding the team in developing safety plans, and ensuring that care is delivered in a culturally competent and strength-based manner.
