Requirements

Current RN or LPN license in good standing.
Minimum of 2 years of nursing experience in long-term care or a related setting.
Strong clinical assessment and documentation skills.

Nice To Haves

Wound care certification is preferred but not required.

Responsibilities

Identify, assess, and treat a variety of skin conditions, including pressure injuries, diabetic, venous, and arterial ulcers, and traumatic or complex wounds.
Complete thorough wound assessments upon discovery and conduct follow-up evaluations weekly or as needed.
Provide direct wound care in accordance with physician orders, manufacturer recommendations, and established nursing standards.
Collaborate with the interdisciplinary care team to develop, implement, and update individualized care plans.
Document all assessments, treatments, and resident responses accurately in the medical record.
Monitor and ensure consistent implementation of preventive skin care measures (e.g. turning schedules, pressure relief devices, hydration/nutrition, incontinence care).
