About The Position

The wound care nurse provides wound care for patients using acceptable wound care and management standards of practice.

Requirements

  • Current nursing license in good standing with the Virginia Department of Health Professions.
  • Certification as a wound care nurse, mandatory.
  • Clinical knowledge and skills necessary to provide wound care treatment and preventive measures.
  • Excellent customer service and leadership skills.
  • Current CPR Healthcare Provider certification.

Responsibilities

  • Reviews all patient admission assessments to ensure skin risk assessments have been completed and appropriate interventions implemented.
  • Completes a body assessment on each new patient admission, implementing any new preventive or treatment interventions as indicated by the assessment.
  • Reviews PCC Dashboard daily for new skin area and ensures areas have been assessed by a nurse, and appropriate prevention and treatment interventions have been implemented.
  • Partners with nursing leadership to ensure body audit and Braden Scale Assessments are completed as scheduled and appropriate nursing follow-up has been implemented.
  • Measures all wounds weekly.
  • Initiates and performs wound care following center protocols and physician orders.
  • Completes accurate and timely documentation of wound and skin assessments according to center policies, but no later than weekly.
  • Communicates regularly with patients and families to update on wound and skin conditions, reporting any new wounds or skin areas found.
  • Implements physician orders for any new treatment and/or prevention orders received.
  • Ensures wound care plans are implemented and updated in a timely manner to accurately reflect the patient’s condition and needs.
  • Collaborates with MDS Coordinators to ensure supporting documentation is available for accurate coding.
  • Participates in the weekly At-Risk meeting to ensure appropriate interventions and/or treatments for any patient identified at risk for compromised skin integrity is/are implemented.
  • Participates in the QAA committee and reports wound trends monthly and/or quarterly, collaborating with the interdisciplinary team in quality improvement activities.
  • Participates in the daily clinical leadership and department director meetings according to CCR protocols.
  • Participates in CNA education for alterations in skin integrity identification and wound care prevention.
  • Participates in nursing education and orientation for prevention and treatment practices.
  • Keeps current professional standards of practice for wound care prevention and treatment.
  • Attends periodic professional meeting and training opportunities offsite for ongoing education.
  • Rounds with wound care physician, physician extender and/or other Center leadership to assess in-house wounds and revise plans of care accordingly.
  • Collaborates with nursing leadership and medical supply coordinator at least monthly to assess support surface usage, evaluate ongoing need for continued use and accuracy of wound care supplies/charges.
  • Performs other duties, as assigned.
