About The Position

Wound Care Nurse (RN) - Skilled Nursing / Post-Acute The Center at Centennial is seeking a dedicated and experienced Wound Care Nurse (RN) to join our compassionate team in a skilled nursing and post-acute setting. We are committed to providing exceptional, patient-centered care that promotes healing and recovery. As a Wound Care Nurse, you will be responsible for providing specialized wound care services to our residents. This includes conducting comprehensive wound assessments, developing and implementing individualized treatment plans, performing advanced wound care procedures, monitoring healing progress, and educating patients, families, and staff on proper wound care techniques. You will play a vital role in our interdisciplinary team, ensuring optimal outcomes for our patients. Key Responsibilities: Conduct comprehensive wound assessments for acute and chronic wounds. Develop, implement, and evaluate individualized wound care treatment plans. Perform advanced wound care procedures and dressing changes. Monitor wound healing progress and adjust care plans as needed. Educate patients, families, and staff on proper wound care techniques and prevention strategies. Maintain accurate and timely documentation of all wound care activities. Collaborate effectively with physicians, therapists, and the interdisciplinary team. Qualifications: Current and unrestricted Registered Nurse (RN) license in the state of practice. Minimum of [e.g., 1-2] years of experience in wound care, preferably in a skilled nursing or post-acute environment. Wound care certification (e.g., WCC, CWCN) is highly preferred.
