About The Position

We are seeking a dedicated and experienced Wound Care Nurse to join our interdisciplinary healthcare team. This position offers the option to work 4 10-hour shifts or 5 8-hour shifts. Under limited supervision, the Wound Care Nurse provides comprehensive assessment, planning, and implementation of care related to wounds, ostomies, and urinary continence across all age groups (neonates to adults). This role supports evidence-based practices and collaborates closely with nursing management and medical staff to improve patient outcomes and promote best practices in wound care throughout the organization.

Requirements

  • Bachelor's degree in Nursing (BSN) from an accredited college or university.
  • Minimum of two (2) years of relevant clinical experience in wound care, ostomy management, staff development, or quality initiatives.
  • Active Registered Nurse (RN) license in South Carolina or a compact state.
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA).

Nice To Haves

  • Supervisory experience is preferred.
  • Strong knowledge of healthcare trends, patient-centered care, and interdisciplinary collaboration.
  • Familiarity with consultative methodologies and patient care operations.

Responsibilities

  • Communicate daily with CNAs, charge nurses, and supervisors regarding skin conditions, treatment changes, and preventive care needs.
  • Implement physician-ordered treatments (ointments, dressing changes, etc.) and document each treatment in real time.
  • Ensure accurate and timely documentation of all skin-related issues in the EMR (Epic), including measurements and descriptions.
  • Collaborate with RNs on staging/classifying wounds and evaluating treatment effectiveness, adjusting as needed in coordination with physicians.
  • Conduct daily and weekly skin audits based on patient skin risk scores; assist with skin assessments on admissions or readmissions.
  • Prepare and submit the weekly Pressure Ulcer Report to the Director of Nursing and primary nurse consultant.
  • Maintain treatment carts and supplies, ensuring proper labeling and disposal of outdated materials.
  • Support development and implementation of individualized patient care plans in coordination with the care team.
  • Assist with direct patient care as needed and report any changes in patient condition to medical staff promptly.
  • Attend and participate in required in-service training, staff meetings, and daily QI (Quality Improvement) stand-up meetings.
  • Document nursing notes in the EMR and demonstrate competency using the clinical software system.
