We're hiring a Registered Nurse (RN) Care Coordinator for Wound Care to join our innovative Value-Based Care team. In this role, you'll serve as a clinical leader in coordinating and managing care for patients with complex wounds, ensuring the right care, at the right time, in the right setting. About the Role as an RN Care Coordinator, you'll be a cornerstone of our VBC strategy triaging patient needs, closing care gaps, and closely collaborating with physicians, specialists, and fellow VBC team members. This position has a specialized focus on supporting patients with chronic or complex wounds, such as diabetic ulcers, pressure injuries, surgical wounds, and skin grafts. You'll lead efforts in patient outreach, population risk stratification, and process improvement, including for high-risk patients with wound care needs.
