Registered Nurse (RN) Care Coordinator Wound Car. Are you a clinically skilled RN with a background or strong interest in wound care? Do you want to make a meaningful impact on high-risk patients in a collaborative, forward-thinking care model? We're hiring a Registered Nurse (RN) Care Coordinator for Wound Care to join our innovative Value-Based Care team. In this role, you'll serve as a clinical leader in coordinating and managing care for patients with complex wounds, ensuring the right care, at the right time, in the right setting. About the Role as an RN Care Coordinator, you'll be a cornerstone of our VBC strategy triaging patient needs, closing care gaps, and closely collaborating with physicians, specialists, and fellow VBC team members. This position has a specialized focus on supporting patients with chronic or complex wounds, such as diabetic ulcers, pressure injuries, surgical wounds, and skin grafts. You'll lead efforts in patient outreach, population risk stratification, and process improvement, including for high-risk patients with wound care needs.

  • Perform or coordinate advanced wound care procedures and treatment plans, including debridement, wound dressing selection, and infection prevention.
  • Lead efforts in the ongoing monitoring and management of patients with chronic or non-healing wounds.
  • Coordinate with specialists (e.g., podiatry, vascular, dermatology) and services such as home health, SNFs, and DME providers to support wound healing.
  • Close care gaps with focused visits that address quality metrics and improve health outcomes.
  • Triage patient calls, referrals, and messages with clinical urgency and empathy.
  • Conduct virtual or in-person assessments to evaluate wound severity, healing trajectory, or risk of infection.
  • Use evidence-based protocols to escalate care when appropriate and prevent avoidable complications.
  • Communicate care plans and wound management strategies to patients clearly and compassionately.
  • Coordinate timely follow-up appointments, referrals, imaging, or home health services as needed.
  • Track wound progress and support patient adherence to care plans and supply needs.
  • Collaborate with PCPs, specialists, and Value-Based Care Navigators to ensure smooth care transitions.
  • Contribute wound care insights during interdisciplinary team meetings and care planning sessions.
  • Document all wound assessments, interventions, and education within the EHR with precision.
  • Support improvement of quality measures tied to wound-related care (e.g., diabetic foot exams, pressure injury prevention).
  • Lead or support process improvement initiatives focused on wound healing outcomes.
  • Provide clear, customized education to patients and caregivers on wound care management, dressing changes, early signs of infection, and proper skin care.
  • Empower patients to take ownership of their wound healing plan and overall health.
  • Required: Associate Degree in Nursing; Preferred: Bachelor of Science in Nursing (BSN) Licensure/Certification
  • Required: Active RN license (State of Utah)
  • Required: BLS Certification
  • Required: 1+ year of direct patient care experience (ambulatory, home health, ED, SNF, or hospital)
  • Strong clinical judgment and wound management decision-making
  • Excellent communication and patient education abilities
  • Proficient in EHR systems, documentation, and tracking wound progression.
  • Familiar with value-based care principles and quality improvement
  • Able to work independently while collaborating with multidisciplinary teams.
  • Preferred: Wound care certification (e.g., WCC, CWCN, or equivalent) or willingness to pursue Experience
  • Highly preferred: Experience or demonstrated interest in wound care, skin grafts, and ulcer management
  • Preferred: Background in care coordination, triage, or population health
