About The Position

NIVA Health is hiring a Wound Care Provider in the Saginaw area to deliver advanced wound care through a facility-based rounding model. You’ll work in partnered clinical settings such as skilled nursing facilities, rehab centers, and assisted living communities, collaborating with on-site care teams to improve healing outcomes and patient quality of life. Requirements

  • Active Nurse Practitioner/Physician Assistant (NP/PA) license.
  • Strong independence, communication, and organizational skills.
  • A passion for patient-centered care.
  • Clinical experience in wound care, SNF/rehab, home health, or related settings is a plus.
  • Strong clinical judgment and communication skills.
  • Reliable transportation for local travel between facilities.

Nice To Haves

  • Wound Care Certification (preferred, with CEUs provided if needed within 90 days of hire).
  • Experience with wound documentation and treatment planning

Responsibilities

  • Provide comprehensive wound assessments and treatment plans
  • Deliver advanced wound care services, including dressing selection, treatment protocols, and ongoing monitoring
  • Perform debridement and other procedures within your scope and credentialing (training and support available)
  • Coordinate care with facility nursing teams and leadership
  • Document visits clearly and efficiently in our system
  • Communicate recommendations and follow-up plans with the care team
