About The Position

We are seeking dedicated and compassionate Wound Care Nurses (LPN or RN) to join our growing healthcare team. This is a high-impact role focused on providing expert wound care in a home health setting. The ideal candidates will have prior wound care experience, strong clinical judgment, and a passion for patient-centered care. As a Wound Care Nurse, you will assess, plan, and implement care for patients with complex wounds, ensuring the highest standards of treatment and compassionate service. You will play a critical role in patient education and collaborate with a multidisciplinary team to support healing and improve quality of life. Requirements

  • Active nursing license (LPN or RN) in the state of practice.
  • Wound care experience required (home health experience strongly preferred).
  • Strong understanding of wound healing principles and infection prevention.
  • Familiarity with DME (Durable Medical Equipment) related to wound care.
  • Ability to travel between patient homes (reliable transportation required).
  • Excellent communication and documentation skills.
  • Compassion, flexibility, and a commitment to high-quality patient care.
  • Must be willing to drive through a designated territory as well as have wound care experience.

Nice To Haves

  • Additional experience with pediatric patients, tube feeding, or medical-surgical nursing is a plus.

Responsibilities

  • Conduct thorough assessments of wounds and patients’ overall health status.
  • Develop and implement individualized care plans.
  • Perform wound care treatments including dressing changes and application of topical agents.
  • Monitor healing progress, watching for infection or complications.
  • Educate patients and families on wound care, prevention, and self-management.
  • Collaborate with other healthcare professionals to coordinate care and optimize outcomes.
  • Accurately document assessments, treatments, and patient progress.
  • Provide care for all home health patients within scope of practice.
  • Stay current with best practices and advancements in wound care.
