About The Position

Wound Care Nurse Essential Job Functions, Duties, and Responsibilities: Assessing, developing treatment plans and monitoring wounds. Identify, manage, and treat specific skin disorders. Working collaboratively with other members of the resident care team to determine if additional treatments or changes in care are required. Examine the resident and his/her records and charts, and identify normal verses abnormal findings. Notification of the resident status to POA and provider for evaluation, supervision and orders for care treatments. Ongoing communication as long as wound is present. Educating residents and care team on prevention, wound care, infection and injury prevention, and pressure ulcer care. Rounding on pressure ulcer/non-pressure ulcer residents weekly to assess wound status, prevention interventions, care planning, notifications and treatment changes needed. Complete timely documentation in electronic records. Including progress notes, care plans, assessment per policy. Reviews physician’s orders to insure accuracy of treatment administration. Recognizes safety hazards, addresses them and reports them to supervisor and care team. Provide logs weekly and review at weekly skin Interdisciplinary Team (IDT) meeting. Notification of other disciplines for collaboration on care concerns ex: Dietician. Other duties as assigned. Required Skills and Qualifications: Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism. Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be able to work with ill, disabled, elderly, and emotionally upset people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families. Education and Experience: Must meet all applicable state and federal requirements for this position. Associates or bachelor’s degree in nursing from an accredited college or university. Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment. Prior wound care experience preferred. Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility Supervisory Responsibility: This position does not have any direct reports or supervisory requirements. Physical Demands: Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals. Physical Requirements - The Physical activities of this position involve: Prolonged periods of standing Ability to lift fifty (50) pounds. Moving, lifting or transferring of residents may involve lifting up to 75 pounds Ability to exert maximum muscle force to lift, push, pull or carry patients Ability to use abdominal and lower back muscles over time without fatigue Ability to stand for extended periods of time Ability to bend, stoop, stretch, twist, sit and reach Fine motor skills Cognitive Requirements - The Cognitive activities with or without prosthetics of this position are: Executes tasks independently Ability to express yourself clearly and effectively verbally Ability to concentrate on moderate details with frequent interruptions Visual acuity Hearing alertness Environmental Requirements - An individual in this position may be exposed to: Ambient room temperatures and lighting Other Requirements - n/a Conclusion: All employees are expected to follow all policies and procedures.
