PURPOSE: The overall supervision of resident skin and wound care services in coordination with the attending physician, the Director of Nursing, or the Medical Director of the facility, with the emphasis on treatment and therapy of wound disorders and cure. You will also assist in modifying the treatment regimen to meet the physical and psychosocial needs of the resident, in accordance with established medical practices and the requirements of the State and the policies and goals of this facility. In addition, the Wound Care Nurse will develop in conjunction with the Medical Director, Director of Nursing and Administrator a comprehensive skin and wound care program. The program will include a special emphasis on diabetic related skin and wound care issues. This program will be coordinated with the Staff Educator. ESSENTIAL FUNCTIONS: In addition to performing Essential Functions, this position may be required to perform a combination of supportive functions based upon the requirements of the organization. Provide assessment and diagnostic services related to wound care. Perform an assessment evaluation using techniques including observation, inspection, and palpation. Conduct weekly skin & wound assessments including pictures. Ensure that residents receive appropriate treatment. Give direct physical and psychological nursing care in emergencies, as well as in the presence of illness or disability to maintain life, provide comfort, reduce stress, and enhance the resident’s ability to cope. Assist in resident rehabilitation activities as it relates to wound care. When required, perform range of motion assist in ambulation, provide emotional support. Monitor the results of physical therapy activities. Provide preventive health care services. Make recommendations to the resident concerning nutrition, prevention of common skin disorders etc. Provide information to resident and family members of prevention techniques and other ways to Maintain or improve the general health and psychosocial status of the resident. In emergency cover for the charge, medication, and treatment nurse. All staff is required to always follow all safety regulations and precautions. Staff must ensure that their department is maintained in a clean and safe manner by assuring that all necessary equipment and supplies are maintained. Report all hazardous conditions/equipment to immediate supervisor/manager Report all accidents/incidents to immediate supervisor/manager within 24hrs All staff must wear protective clothing and equipment when handling infectious waste and or blood/body fluids or when doing Hazardous work i.e., working with chemicals, electrical or day to day maintenance repairs. All Staff are required to participate in the Person-Centered Care - Managers/Supervisors will help staff in assisting in participating in Person Centered Care. Recognize your role as part of the QAPI efforts of your organization. Attend trainings to build understanding and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned. Follow established policies and procedures in support of QAPI efforts. Look for and share with leadership ideas for improvement in the organization. Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
