About The Position

We are a dynamic mobile medical practice serving patients in private homes, nursing homes, and assisted living facilities throughout Livingston and surrounding areas. Our mission is to deliver compassionate, high-quality care directly to patients where they live, with a strong focus on wound management and geriatric wellness.
We are seeking a Wound Care Nurse Practitioner (NP) to join our growing team. This role is ideal for a clinician who thrives in autonomous settings, values patient-centered care, and is passionate about improving outcomes for individuals with acute and chronic wounds.

Requirements

• Active Nurse Practitioner license in New Jersey
• Minimum 1 year of wound care experience (home health or facility-based a plus)
• Strong clinical judgment and ability to work independently
• Excellent communication and documentation skills
• Valid driver’s license and reliable transportation

Nice To Haves

• Certification in Wound Care (WOCN, WCC, or equivalent) preferred

Responsibilities

• Perform comprehensive wound assessments and develop individualized treatment plans
• Provide advanced wound care including debridement, dressing changes, and infection management
• Collaborate with physicians, caregivers, and facility staff to coordinate care
• Document visits and treatment plans accurately in EMR systems
• Educate patients and caregivers on wound prevention and care
• Travel to patient locations within Livingston and nearby communities
