About The Position

The HCHB Workflow Specialist will perform all necessary home health workflow clinical tasks for multi-site locations to ensure the timely review, edit and flow of tasks within HCHB for all disciplines on the home health team. Requirements

  • Must be a graduate of an approved school of professional nursing with a current license in the state of Arizona as a Registered Nurse.
  • Must have a minimum of one-year home health nursing experience.
  • Computer knowledge is required with prior experience in HCHB preferred.
  • Must be organized, detail oriented and possess effective communication skills, both orally and in writing.
  • The ability to communicate with a diversity of individuals is required.
  • Must possess a valid driver’s license.
  • Must possess automobile liability insurance.
  • Must possess a current and valid Level One Fingerprint Clearance Card
  • Communicate effectively with staff, patients and/or family through speaking and hearing.
  • Manual dexterity to operate office equipment including, but not limited to, computers, printers, faxes and/or copiers.
  • Visual acuity to evaluate and peruse employee documentation.
  • Sedentary physical demands include but not limited to sitting for prolonged periods.

Nice To Haves

  • Medicare home health and management experience is preferred.
  • Computer knowledge is required with prior experience in HCHB preferred.

Responsibilities

  • Review home health Start of Care, resumption of care, Evaluation, discharge, and transfer documentation.
  • Review and verify admission.
  • Review and verify all Initial Plans of Care to ensure adherence to written specifications.
  • Review, edit and lock all home health OASIS items.
  • Review and verify all home health face to face requirements.
  • Review, edit and approve all Recertification’s.
  • Review and approve all physician’s orders.
  • Monitor and verify all visit coordination notes.
  • Ensure Recertification/Discharge decision workflow is processed timely.
  • Utilize current technology to fulfill job functions.
