About The Position

This position reports to the clinic site nurse (LVN), under the direction of the director of clinic operations, and works closely with OB/Gyn providers and staff. This role supports Women’s Health patients throughout their care OB/Gyn care by coordinating scheduling, referrals, education, follow-up, and administrative needs. The navigator functions as a communication hub, strengthening care collaboration and ensuring patient-centered outcomes.

Requirements

  • High school graduate or equivalent.
  • Current BLS Certification by American Heart Association or American Red Cross.
  • Medical Assistant certification required.
  • Minimum of five (5) years of OB/GYN experience.

Responsibilities

  • Answers the telephone properly using department name and personal first name slowly and clearly.
  • Records and delivers messages accurately in the EHR system and in a timely manner to staff and providers.
  • Is knowledgeable in the use of PC and applications specific to the department. Demonstrates proficiency with the EHR and related workflows for timely data collection and documentation.
  • Demonstrates comprehensive knowledge of public health programs offered by the clinic (e.g., Every Woman Counts, Family PACT, Medi-Cal, CHDP, Connect to Care, CPSP) and accepted insurance plans, including eligibility requirements, and guides uninsured patients to appropriate programs and services.
  • Proficient in the use of systems to verify insurance eligibility, benefits, and co-pays with knowledge of downtime procedures when systems are unavailable.
  • Collaborates with the interdisciplinary team to develop and implement new programs and services that address the evolving needs of women’s health.
  • Adheres to clinic and hospital policies, guidelines, and regulatory standards, including HIPAA.
  • Conducts annual protocol reviews in partnership with the OB Medical Director and the Clinic Operations Director to ensure compliance and identify areas for improvement.
  • Collaborates with care teams to address patient concerns, foster trust, and improve appointment attendance with a focus on optimizing health outcomes.
  • Provides individualized patient care and education throughout pregnancy, coordinating nutrition, psychosocial, and educational needs. Monitors risk factors, ensures appropriate referrals and interventions, and follows established protocols approved by the supervising physician. Utilizes educational materials such as pamphlets, videos, and other approved resources.
  • Delivers comprehensive health education to promote positive pregnancy outcomes, including topics such as nutrition, genetic testing, women’s health, family planning, and routine screenings. Enhances patient engagement by providing guidance on prenatal classes, postpartum support, and contraceptive counseling.
  • Conducts prenatal intake (orientation), explaining procedures, distributing lab orders, collecting history, reviewing pregnancy plans, and scheduling follow-up appointments.
  • Reviews and maintains perinatal education materials and ensures adequate supply.
  • Ensures timely availability of lab results, imaging studies, physician consults, and other testing for provider review.
  • Recognizes urgent medical issues and promptly notifies the site nurse or provider.
  • Demonstrates proficient use of Language Line or approved interpretation services. Provides accurate translation (if bilingual) to ensure patient understanding.
  • Monitors assigned patients’ attendance, referrals, testing, and barriers to care. Reduces gaps in care by tracking missed appointments, follow-up treatments, and test results.
  • Support high-risk pregnancies by tracking care plans, coordinating specialist referrals, and assisting in case management by coordinating all ancillary testing appointments.
  • Coordinates all women’s health referrals, including imaging, lab testing hospital pre-registration, maternity tours and referrals to external specialists.
  • Serves as a non-clinical point of contact for patient navigation and concerns.
  • Ensures proper documentation and tracking for quality programs (prenatal/postpartum measures, cancer screenings).
  • Assists with review of patient chart prior to pregnancy closure and ensures transfer of prenatal records to the hospital prior to delivery.
  • Assists providers with inputting charges as needed.
  • Demonstrates correct scheduling procedures for outpatient services, provides instructions, and verifies completion of services.
  • Identifies barriers related to social determinants of health (SDOH) and connects patients with resources.
  • Demonstrates knowledge of proper procedure for obtaining outside medical records, completing paperwork, and verifying receipt.
  • Meets productivity standards and reports variances to supervisor (overtime, incomplete tasks, patient care issues).
  • Communicates patient flow issues and delays to supervising nurse, providers, and patients.
  • Completes patient forms (EDD, FMLA, supplemental insurance, etc.) and ensures provider sections are addressed.
  • Keeps patients informed of delays and ensures courteous communication at all times.
  • Serves as a knowledgeable resource to all of the OB/GYN providers when needed.
  • Performs all other duties as assigned by provider or supervising site nurse within scope of practice.
